Evaluates, plans and implements a goal oriented Speech Pathology treatment program that focuses on speech-language disorder, swallowing impairments/dysfunction and cognitive functioning to achieve an optimal level of independence. Provides an environment conducive to functional goal achievement. Utilizes interdisciplinary collaboration and available resources to provide care.

Responsibilities

A day in the life of a Speech Therapist at Hackensack Meridian Health includes:

 

1. Gathers all information necessary to properly evaluate the patient¿s needs, including testing and consultations with other health care professionals.
2. Formulates functionally based and measurable short term and long term goals.
3. Develops and implements an individualized treatment program that focuses on achievement of the established goals.
4. Participates in the ongoing development of the discharge plan that was initiated at the time of the primary visit.
5. Prepares written documentation such as Progress Notes, Reassessments, and Discharge Summaries.
6. Develops and implements individualized goals and treatment program focused on identified resident needs.
7. Focuses on achievement of the established goals.  Participates in the ongoing development of the discharge plan.
8. Completes patient treatments with a scheduled day.
9. Evaluates effects of treatments by observing, noting, and evaluating patient's progress then recommending adjustments and modifications.
10. Develops a rapport and mutual respect with the patient to foster an optimum environment for goal achievement.
11. Provides education to patients and/or families based upon the identified learning needs in a manner that the patients and/or families can demonstrate their understanding.
12. Establishes appropriate methods of communication to improve the patient¿s independence in his/her interaction with others.
13. Establishes appropriate swallowing compensation to minimize patient risk of respiratory and nutritional compromise.
14. Demonstrates appropriate knowledge in providing for the assessment and remediation of cognitive, visual-perceptual skills and/or behavioral parameters.

Qualifications

Education, Knowledge, Skills and Abilities Required:

1. Perform clinical care professionally
2. Serve as patient advocate and role model to other Team Members
3. Deliver patient care with minimal disruptions and obstacles
4. Ability to effectively work on a multidisciplinary team
5. Dysphagia experience required
6. MBS experience preferred

 

Education, Knowledge, Skills and Abilities Preferred:

1. Computer skills, including Microsoft Office
2. Long-term care facility experience
3. Prior work experience with geriatric and disabled populations.
4. Bi-lingual (Spanish)

 

Licenses and Certifications Required:

1. Speech Language Pathology License.
2. CCC-SLP
3. CPR certification preferred
