Overview

The Observation Nurse Practitioner or Physician Assistant at JFKUMC is responsible for providing competent and comprehensive care to patients and their families throughout the patient's life span. Patient care responsibilities have a specific focus on treatment of acute and chronic care issues including prenatal care, health education and maintenance, disease prevention, and continuity of care. The APN actively participates in the multi-disciplinary approach to health care and views the individual and family as an integral part of that team. The APN supports and implements the philosophy, goals and standards of JFK Medical Center. Active member in a patient centered-team approach in the care of the patient and families. Works collaboratively with all team members in achieving shared goals with a consistent focus on quality care. Participates in quality improvement activities including data analysis, implementing recommendations, evaluating changes and outcomes.

This is a NIGHT, 36 hr/week position.

Responsibilities

1. Obtains a thorough patient history that includes: Determination of their health status and primary care service needs. Analysis of primary symptoms. Assess patient compliance with historical primary care treatment plan if one exists. Complete medical and social history. Performs an appropriate physical examination and recognizes normal and abnormal findings. Records findings of the physical exam in a clear and concise manner using the appropriate format. Connect patient with a primary care physician based on the data gathered in the history and physical examination. Counsel, educate, and instruct patients in specific disease-related and preventative medicine areas.

 

2. Communicates with assigned Primary Care physician regarding pertinent patient information including but not limited to actual or potential clinical problems. Collaborates with physicians, family members, social workers, community contacts and other members of the health care team in patient care planning.

 

3. Utilizes assessment skills and collaborates with other disciplines to develop a comprehensive education plan to meet patient/family needs. Utilizes assessment skills and collaborates with other disciplines to develop a comprehensive plan of care that meets patient needs. Incorporates diagnosis, interventions, and expected outcomes. Communicates plans of care with the patient and family. Identifies the patients educational needs based on the assessment of the patient.

 

4. Understands his/her role as a professional and conduct him/herself in accordance with the same. Seeks out other professionals to network and create positive alliances. Is committed to a positive meaningful interaction with the patients. Will oversee Community Outreach workers, and conduct weekly meetings to discuss patient progress, goals and issues/concerns.

 

5. Other duties and/or projects as assigned.

 

6. Adheres to HMH Organizational competencies and standards of behavior.

Qualifications

Education, Knowledge, Skills and Abilities Required:

1. Master's Degree in Nursing with Advanced Nurse Practitioner credentials from an accredited Nurse Practitioner Program required.

2. Demonstrates effective communication and leadership skills including teaching principles, organizational and presentation skills. Computer skills with a working knowledge of electronic medical records required.

3. Excellent written and verbal communication skills.

4. Proficient computer skills that may include but are not limited to Microsoft Office and/or Google Suite platforms.

 

Education, Knowledge, Skills and Abilities Preferred:

1. Acute hospital experience preferred.

2. Ambulatory care experience preferred.

 

Licenses and Certifications Required:

1. Advance Practice Nurse License.

2. AHA Basic Health Care Life Support HCP Certification.

3. C.D.S. (Controlled Dangerous Substance) registration is required for APNs who will be prescribing controlled dangerous substances.

4. D.E.A. (Drug Enforcement Agency) certification is required for APNs who will be prescribing controlled dangerous substances. 

 

Knowledge, Education and Skills Required:  

  • Successful completion of an approved physicians assistant program, accredited by the Accreditation and Review Committee for Physician Assistants (ARC-PA) or other NCCPA certification examination  qualifying accrediting agency.
  • Maintenance of a minimum of 100 hours of CME in accordance with NCCPA regulations every 2 years

 Knowledge, Education and Skills Preferred:  

    • 2 years of thoracic surgery experience is preferred however new graduates are considered.
    • Preferred robotic training

 Required Certification/Licensing:    

  • New Jersey State Physician Assistant Certification 
  • Basic Cardiac Life Support Certification required
  • Advanced Cardiac Life Support Certification
  •  New Jersey CDS license
  • Federal DEA license
