The Process Coordinator provides direct administrative assistance to the Coordinated Care staff as it relates to the admission, discharge and denial process. Coordinates department workflow with use of computerized system. Acts as a liaison for patient and/or family, performing financial/social screens for discharge planning purposes. Coordinates and maintains data collection for performance improvement initiatives. Provides direct administrative assistance with the handling of incoming and outgoing correspondence.

Responsibilities

A day in the life of a Process Coordinator at Hackensack Meridian Health includes:

  • Reviews all admitted patients by unit assignment via computerized system and communicates essential information to appropriate staff. Reviews daily discharge log and verifies discharge dates. Inputs discharge disposition into computerized system.
  • Collaborates with physician advisor, appropriate case manager, and denials team to communicates denial information . Prepares all correspondence for denials for HQSI including but not limited to, termination of benefits and lower levels of care.
  • Reviews and distributes RAC requests to physician advisors. Prepares all correspondence for DCS, Highmark, Maximus, and AJL. Maintains level of appeal log to ensure timely correspondence. Utilizes NJHA computerized appeals systems.
  • Assigns all level of care determinations in computerized system as assigned by physician, staff, and/or insurance companies.
  • Performs social/financial screen for patient/family as referred by staff. Reviews, monitors and maintains daily insurance logs.
  • Prepares denial correspondence as needed.
  • Maintains and updates HQSI agreement and disagreement statistics. Prepares reports for physician advisors as needed.
  • Other duties and/or projects as assigned.
  • Adheres to HMH Organizational competencies and standards of behavior.

Qualifications

Education, Knowledge, Skills and Abilities Required:

  • Associate Degree in Psych/Social preferred. High School diploma, general equivalency diploma (GED), and/or GED equivalent programs with equivalent job related experience considered.
  • Acute care hospital experience, medical terminology, registration/insurance knowledge and computer skills are a plus.
  • Computer skills including but not limited to Word, Excel and Power Point.
