The Nurse Navigator, ICD Triage is an advocate, educator, and coordinator of care for patients who are followed in the Institute for Child Development (ICD). This role will be responsible for triaging patients, navigating them to appropriate therapeutic resources, and participating in developmental assessments. This role has excellent communication and critical thinking skills, a strong understanding of mental and behavioral health, and a passion for connecting patients with the care they need.

 

Responsibilities

A day in the life of a Nurse Navigator, ICD Triage at Hackensack Meridian Health may include:

 

  • Reviews intake packets to determine appropriate referrals and urgency of care required.
  • Guides intake staff to schedule patients in appropriate visit spots.
  • Connects patients with therapists and diagnoses specific resources based on their individual needs and preferences.
  • Facilitates referrals and coordinates care follow-ups and transitions.
  • Assists in age-appropriate developmental screenings and assessments for children and adolescents.
  • Score assessment and communicate findings to providers.
  • Works collaboratively with physicians, APNs, nurses, MAs, intake staff, and other members of the healthcare team to ensure coordinated and comprehensive care.
  • Maintains accurate patient records and documentation.
  • Adheres to all relevant regulations and ethical guidelines.
  • Identifies the needs of the patient population served and modifies and delivers care that is specific to those needs (i.e., age, culture, hearing and/or visually impaired, etc.). This process includes communicating with the patient, parent, and/or primary caregiver(s) at their level (developmental/age, educational, literacy, etc.).
  • Provides effective education and support to patients and their family members.
  • Other duties and/or projects as assigned.
  • Adheres to HMH Organizational competencies and standards of behavior.

Qualifications

  • BSN degree
  • Minimum of 5 or more years of experience in pediatric clinical experience.
  • NJ State Professional Registered Nurse License.
  • BLS AHA Basic Health Care Life Support HCP Certification.
  • Ability to work independently and also collaborate with members of the ICD team.
  • Strong organizational, time management, critical thinking skills.
  • Excellent written and verbal communication skills.
  • Proficient computer skills that include but are not limited to Epic, Microsoft Office, and/or Google Suite platforms.
  • Nurse Navigator Certification preferred 

Starting Minimum Rate

Starting at $111,924.80 Annually

Some jobs may also be eligible for performance-based incentives, bonuses, or commissions not reflected in the starting rate. Certain positions may also be eligible for shift differentials for work performed on evening, night, or weekend shifts.
