The Community Outreach RN Coordinator is responsible for coordinating and implementing Hackensack Meridian Health's Community Outreach Wellness Plan to address identified community health needs and in support of Hackensack Meridian Health's strategic imperatives and centers of excellence. Implements and evaluates health education programs, and coordinates the provision of health education services.

Responsibilities

A day in the life of a Community Outreach RN Coordinator at Hackensack Meridian Health includes:

  • Implement assigned outreach activities according to the community outreach plan. Ensure all logistics related to health screenings, health fairs, education programs and special events are completed in a timely, efficient and accurate manner.
  • Represent Hackensack Meridian Health Community Outreach department on boards, coalitions and committees that are strategically aligned with the community benefit plan. Participates on appropriate committees and boards as directed by the Manager. Reports and communicates meeting information and follow up items to the department as needed.
  • Identify and maintain relationships with key community contacts who share the mission for improving health within the Hackensack Meridian Health communities we serve and that support the Community Health Needs Assessment Implementation Plan.
  • Coordinates nursing and health promotion services based upon community need. Assesses and identifies populations at risk.
  • Accurately collects and inputs data into outreach database or other system on a regular basis as directed. Submit monthly management report summarizing outreach activities and outcomes. Provides program attendance, event summary and evaluation forms for events per protocol.
  • Coordinates with nurses, health educators, community members, physicians and other hospital network employees in the scheduling of health related activities. Organize and schedule other health and medical professionals to present health education and screening programs
  • Gathers and shares community/public health information on their assigned territories. Keeps current with trends in public/community health needs
  • Communicates effectively with health professionals, community members and organizations.
  • Supports the research process and utilizes the information to support evidence based care. Actively seeks opportunities to incorporate new scientific finding into education programs and/or clinical practice unit in area of expertise.
  • Independently develops and evaluates a personal continuing education and professional development plan.
  • Provides orientation, and in-service for new nursing staff.
  • Referring individuals who have screening results that fall outside the normal parameters to their primary care physicians, appropriate health care providers or the Hackensack Meridian Health physician referral line.
  • Identifying socioeconomic conditions which relate to or affect general health and well-being and make referrals to appropriate agencies. Interprets available services and communicates them effectively
  • Administer social determinant of health (SDOH) surveys to community members at appropriate events and makes referrals via NowPow as needed.
  • Provide health promotion services in a variety of settings which requires lifting and moving equipment, tables, chairs as well as walking, bending, standing and stair climbing.
  • Successfully completes yearly department competencies as assigned. Completes Health Stream modules on time as assigned.
  • Other duties and/or projects as assigned.
  • Adheres to HMH Organizational competencies and standards of behavior.

Qualifications

Education, Knowledge, Skills and Abilities Required:

  • Graduation from a School of Nursing approved by the NJ State Board of Nursing.
  • Minimum of 3 to 5 years nursing experience.
  • Willing and able to travel.
  • Excellent written and verbal communication skills.
  • Proficient computer skills that include but are not limited to Microsoft Office and/or Google Suite platforms.

Education, Knowledge, Skills and Abilities Preferred:

  • Bachelor of Science in Nursing (BSN).
  • Bilingual.
  • Previous community health experience.

Licenses and Certifications Required:

  • Current Valid NJ State Professional Registered Nurse's license.
  • Maintains a current driver's license.
  • Annual CPR certification.

Starting Minimum Rate

Starting at $95,555.20 Annually

Some jobs may also be eligible for performance-based incentives, bonuses, or commissions not reflected in the starting rate. Certain positions may also be eligible for shift differentials for work performed on evening, night, or weekend shifts.
