Hackensack Meridian Health Carrier Clinic’s East Mountain Youth Lodge provides a continuum of out of home residential treatment services for adolescents between the ages of 13-18. We provide 24 hour clinical, therapeutic treatment for emotional, behavioral, psychiatric and substance abuse difficulties based on the resident strengths and needs.

 

The Case Manager, East Mountain Youth Lodge (EMYL) at Carrier Clinic provides administrative case management support to the clinical and leadership teams of EMYL in maintaining the proper clinical documentation, various signed consents, and psyc-educational plans for residents of EMYL. This administrative case management support will help ensure compliance to the New Jersey Department of Children and Families (DCF), Division of Children's System of Care (CSOC) licensing standards for Intensive Residential Treatment Services (IRTS), Residential Treatment Center & Behavioral Health/Substance Use (RTC & RTC/BH/SU), and Psychiatric Community Home (PCH) programs at EMYL.

 

Responsibilities

1. Case managers will coordinate daily with leaders in their assigned areas for daily assignments, as well as carry out standing duties.

2. Case managers will have significant communication with internal departments and outside treatment partners and families.

3. Case managers will have specific duties according to their assigned role as follows:

 

Admissions Case Management Responsibilities

  • Schedule meet and greets new residents and families
  • Answer phone calls in real time
  • Facilitate pre-admission processes / paperwork
  • Facilitate admissions into the East Mt. Youth Lodge

Nursing Case Management Responsibilities

  • Insurance verifications/referrals
  • Coordination of vaccine appointments and tracking for residents
  • Assistance with scheduling resident lab testing needs
  • Guardian outreach for medication consents
  • Assist with management of nursing groups
  • Medication room audit
  • Scheduling routine appointments

Clinical Case Management Responsibilities

  • Consents- verifying all consents at intake and obtaining new ones
  • Discharge planning paperwork
  • Coordinating any meet and greets
  • Receiving and processing paperwork for next placements
  • Communicating with internal and external teams about planning
    • School coordinator and calls for SCESC, processing intake paperwork from guardians 
    • Working on communication with districts and SCESC to work up to confirmed start dates
    • Attend meetings with EMS regarding classified pending students
  • Adolescent Unit communication
    • Email phone list upon admission,
    • AU letters to OOL
    • School re-entry form (with clinical approval)
  • Team phone calls
    • Updates, scheduling treatment meetings if needed
  • Treatment planning
    • Attend Joint Care Review (JCR) treatment meetings to receive treatment updates and complete any non-clinical follow up from the meeting
    • Start JCR and CANS with final completion/approval from the therapist
    • Update phone/visit/pass plans with clinical leadership approval
    • Treatment plan labeling, filing, composing mailing letters/emails, mailing
    • Schedule visits/passes with CMO and guardians

4. Attend leadership meetings for any relevant input and follow up related to care of residents related to administrative case management support.

5. Other duties and/or projects as assigned. 6. Adheres to HMH Organizational competencies and standards of behavior.

Qualifications

Education, Knowledge, Skills and Abilities Required:

  • Bachelor's degree in relevant field with minimum of 3 years of relevant experience or Master's Degree with 1 year of relevant experience.

 

Licenses and Certifications Required:

  • AHA HeartSaver First Aid CPR AED Certification or completed within the first 90 days of employment. 

Starting Minimum Rate

Starting at $25.38 Hourly

Some jobs may also be eligible for performance-based incentives, bonuses, or commissions not reflected in the starting rate. Certain positions may also be eligible for shift differentials for work performed on evening, night, or weekend shifts.
