The Clinical Pharmacist, Population Health, is responsible for identifying opportunities to improve quality and outcomes of care and managing pharmacy expenditures at the population health levels. This individual will provide consultation to and review of the medication regimen of the high-risk populations /patients identified within the Hackensack Meridian Health CIN population. This includes education across the health care system and implementing cost-saving initiatives. The clinical pharmacists will provide innovative pharmacy services to patients served by Population Health. Specifically, in preventing initial inpatient admissions, preventing hospital readmissions, and decreasing inpatient and outpatient spending. The clinical pharmacist shall serve as a resource for care coordination teams and CIN providers.

 

A Pharm.D. with a residency in managed care is preferred.

Responsibilities

A day in the life of a Clinical Pharmacist, Population Health at Hackensack Meridian Health includes:

  • Analyzes and interprets pharmacy and healthcare utilization and cost data within populations to identify opportunities for medication management intervention.
  • Review of payer data to identify opportunities for optimization of medication therapy to improve patient outcomes.
  • Collaborate with network physicians and practice administrators to improve prescribing practices to comply with national standards of quality for medication prescribing.
  • Assists in identifying and developing interventions for HMH attributed members that are not meeting target quality measures.
  • Identify populations at risk. Develop strategic initiatives to further improve medication management, quality outcomes and value-based prescribing outcomes for patients using population health analytics.
  • Identify, implement and monitor programs with external vendors and service providers that support program goals to enhance medication therapy in alignment with Population Health initiatives.
  • Supports the strategic management and initiatives for the optimization of medication use for the Clinically Integrated Network partners.
  • Develop guidelines and protocols to standardize best practice for medication prescribing using evidence-based medicine.
  • Create internal infrastructure and policies to support formulary management to promote value-based prescribing.
  • Evaluate and integrate technological resources to promote improvement in medication adherence and ease access to medication therapy.
  • Promote and facilitate access to clinical pharmacy resources to patients through Medication Therapy Management (MTM) and other novel approaches for comprehensive medication management.
  • Promotes medication management in alignment with care coordination team initiatives in support of value-based practice models.
  • Support the development of clinical pharmacy programs to support population health initiatives for the value-based practice models such as (e.g., reducing avoidable inpatient admissions, reducing avoidable emergency department visits, etc.).
  • Other duties and/or projects as assigned.
  • Adheres to HMH Organizational competencies and standards of behavior.

Qualifications

Education, Knowledge, Skills and Abilities Required:

  • Graduation from an accredited School of Pharmacy with a Doctorate in Pharmacy (PharmD) with 2 or more years of relevant work experience or Bachelor's degree with 5 or more years of relevant work experience.
  • Experience in a clinical pharmacy setting.
  • Experience with Medication Therapy Management & Comprehensive Medication Reviews.
  • In-depth knowledge of pharmacotherapy and pharmacy practice; experience with current medication management guidelines for chronic disease and geriatric conditions, such as chronic obstructive pulmonary disease, congestive heart failure, diabetes, dementia, etc.
  • Must be able to communicate effectively, both oral and written, with care management team, patients, physicians and other health care professionals.
  • Ability to prove technical knowledge of Microsoft Excel; ability to quickly learn Clinical Applications.
  • Experience in developing education curricula and programs for medication management.
  • Experience working within managed care model. 

Licenses and Certifications Required:

  • Registered Pharmacist License.

Starting Minimum Rate

Starting at $1,537.32 Annually

Some jobs may also be eligible for performance-based incentives, bonuses, or commissions not reflected in the starting rate. Certain positions may also be eligible for shift differentials for work performed on evening, night, or weekend shifts.
