The Registered Dietitian for Diabetes serves as a key member of the Comprehensive Diabetes Team of the Pediatric Subspecialty Endocrinology Diabetes Team, providing general diabetes education and diabetes nutrition education in accordance to ADA guidelines. This includes completing comprehensive patient assessments, formulating a plan of care in collaboration with other disciplines, and appropriate and accurate documentation in the medical record.

This state-of-the-art, 60,000-square-foot facility will offer a wide array of services, including primary care, urgent care, advanced imaging, and specialists. You will be joining a visionary project that establishes a new standard for healthcare delivery in suburban areas.

Responsibilities

A day in the life of a Registered Dietitian for Diabetesat Hackensack Meridian Health includes:

  • Completes a comprehensive diabetes educational assessment prior to initiating a plan of care. Education and counseling on diabetes and nutrition is provided to patients and/or family members. This is based on identified learning needs in a manner that demonstrates their understanding.
  • Readiness to learn and barriers to learning are documented in the interdisciplinary education record.
  • Performs diabetes educational assessment and develops a plan of care for patients utilizing ADA guidelines and recommendations.
  • Documents above in accordance with unit-based protocols, established institutional policies and procedures, New Jersey DOH regulations and accreditation agencies.
  • Proficient in the use of computerized educational documentation tool and uses tool to demonstrate ongoing educational progress and continuity of care.
  • Maintains skills in computer software used for Diabetic Care and Education.
  • Contributes effectively as part of the comprehensive diabetes team. Communicates patient findings and shares significant information during the team conference in the comprehensive diabetic clinic service.
  • Reports issues related to process and duties to team leader in order to promote efficiency and effectiveness of the comprehensive diabetes team.
  • Facilitate patient flow and download blood glucose meters as needed. Collaborate with Clinician`s on Continuous Glucose Monitoring Systems including operation, sensor device insertion and downloading/interpretation of monitoring data.
  • Develops curriculum based on ADA standards.
  • Teaches patient centered classes on general diabetes education and diabetes nutrition. Maintains roster of clients. Schedules agreed upon number of educational classes per month. Contacts clients prior to class as a reminder in order to maintain attendance levels and provide services to as many patients as possible.
  • Develops pre and post tests to evaluate effectiveness of instruction provided. Communicates results of pre and post test to manager and team leader on a quarterly basis.
  • Provides accurate education to diabetes clients consistent with AADE7 Self Care Behaviors. Education content includes but is not limited to:
    • Overview of the disease of diabetes, the role of insulin and glucose, and their effects on metabolism
    • Stress management
    • Nutritional management
    • Medication action, peak, onset and side effects
    • Insulin administration and self-testing
    • Sick day management
    • Hypo/hyperglycemic management
    • Foot and skin care
    • Lifestyle modifications.
  • Maintains Commission on Dietetic Registration re-certification requirements on a continual basis. Attends a minimum of one continuing education program annually. Documents professional development activities independently and informs supervisor of status. Maintains certification as a Certified Diabetes Care and Education Specialist.
  • Responsible for ordering educational materials and supplies needed for the comprehensive diabetes clinic, classes, individual instruction, and general use. Maintains appropriate amount of stock on hand. Updates teaching materials as needed and outdated materials. Participates in facility/system-wide community-based activities. Assist in product incorporation and act as industry liaison for Diabetes supplies including meters, pumps, continuous glucose monitoring and insulin products.
  • Promotes patient satisfaction through demonstrated commitment to providing excellent service by interacting in a friendly and helpful manner. Communicates potential patient satisfaction issues to manager and / or comprehensive diabetes team leader.
  • Performance improvement: Completes data collection for PI activities as assigned. Assists in the development and implementation of changes in practice as a result of PI outcomes.
  • Adheres to HMH Organizational competencies and standards of behavior.
  • Lifts a minimum of 5 lbs., pushes and pulls a minimum of 5 lbs. and stands a minimum of 1 hour a day.

Qualifications

Education, Knowledge, Skills and Abilities Required:

  • Registered Dietician in the State of New Jersey
  • Certification as a Certified Diabetes Care and Education Specialists through the American Diabetes Association.
  • Bachelor's Degree
  • One year of Pediatric diabetes-related experience

Education, Knowledge, Skills and Abilities Preferred:

  • Experience with managing patients on insulin pumps and use of CGMS systems. May be required in some departments.

Licenses and Certifications Required:

  • Registered Dietician License.
  • Certified Diabetes Educator.

Starting Minimum Rate

Minimum rate of $78,395.20 Annually

Some jobs may also be eligible for performance-based incentives, bonuses, or commissions not reflected in the starting rate. Certain positions may also be eligible for shift differentials for work performed on evening, night, or weekend shifts.
