Serves as clinical resource to the nursing staff in the development and implementation of nursing care for patients with ostomies, draining wounds, pressure and dermal ulcers and incontinence. Promotes patient satisfaction, utilizing HMH patient satisfaction principles.

Responsibilities

 

  • Serves as primary patient educator to all patients undergoing ostomy surgery.
  • Assesses patients and significant others pre/post operatively in order to promote physical and psycho- social adaptations within 72 hours of the enterostomal procedure unless otherwise ordered by physician.
  • Educates patients and families regarding ostomy care to facilitate rehabilitation through independence and selfcare.
  • Provide input into discharge plans and referrals as needed based on continuing assessment of patient status.
  • Responsible for pre-operative marking of stoma site reflecting assessment of patients anatomy, physical capabilities and life-style.
  • Measures effectiveness of interdisciplinary nursing care plan and recommends adjustments the plan to meet patients changing needs.
  • Serves as clinical resource for the treatment of patients with altered skin integrity due to pressure necrosis, drainage or incontinence. Makes frequent, regular rounds on nursing units. Is available for consultation on a case-to-case basis as well.
  • Works with staff to develop a plan of care to prevent pressure ulcers in high risk hospitalized patients.
  • Works with staff to develop a plan of care for patients with altered skin integrity specific to the type of skin alteration.
  • Assesses patient response to treatment and confers with medical staff about treatment goals or options. Wounds to be re-measured and reevaluated each week with documentation by E.T. and staff nurses.
  • Provides on-going staff education and patient education relative to skin care. Works with nurse representatives from each unit to develop and maintain clinical expertise of staff.
  • Serves as a clinical resource in the management of patients with draining wounds or fistulas. Makes frequent, regular rounds on nursing units. Is available for consultation on a case-to- case is as well.
  • Works with staff to develop a care plan relative to the size and location of wounds and the character of the drainage.
  • Provides in-service education to staff relevant to wound management.
  • Evaluates effectiveness of care and treatment by weekly reevaluation of wound size and characteristics and through conference with medical staff as indicated.
  • Assesses and meets the educational needs of the nursing staff.
  • Provides information as needed about new treatments and procedures related to ostomy, wound care and incontinence.
  • Provides clinical input into the development of wound, ostomy or incontinence policies. Assists in the review and revision of wound, ostomy or incontinence policies in order to meet current standards of practice in each area.
  • Evaluates ostomy and skin care products and supplies on a continuing basis. Initiates cost- effective products when applicable.
  • Reviews stock items and suggest revisions as needed.
  • Participates in research programs relative to E.T. nursing practice, including benchmarking efforts.
  • Demonstrates competency in assessing, analyzing and utilizing data for the purpose of obtaining information necessary for effective decision-making and performance processes.

Qualifications

Education, Knowledge, Skills and Abilities Required:

  • Graduate of an accredited school of nursing with a B.S. in Nursing.
  • Graduate of an approved enterostomal nurse program.
  • Current board certification in E.T. nursing or within one (l) year of hire date.
  • Five (5) years medical-surgical nursing.

 

Licenses and Certifications Required:

  • NJ State Professional Registered Nurse License. 

Starting Minimum Rate

Starting at $45,375.20 Annually

Some jobs may also be eligible for performance-based incentives, bonuses, or commissions not reflected in the starting rate. Certain positions may also be eligible for shift differentials for work performed on evening, night, or weekend shifts.
