The Manager, Social Impact & Population Health will lead and manage the team of Community Health Workers (CHWs) and the Program Coordinator to deliver community based support to high-risk low-income patients to help them reach their health goals.

 

This is a grant funded position.

Responsibilities

A day in the life of a Manager, Social Impact & Population Health at Hackensack Meridian Health includes:

  • Recruit CHWs through engaging with local community organizations.
  • Organize and implement CHW training.
  • Lead a team of CHWs who will work with hundreds of patients each year, assisting patients with getting medical care, setting and achieving health goals, and helping them stay healthy.
  • Support, coach and mentor CHWs to ensure they provide their best work for patients.
  • Create and Analyze reports to ensure CHWs are achieving the program goals.
  • Integrate CHWs with care teams working in clinics and hospitals.
  • Facilitate team meetings and support groups for patients.
  • Manages the operating budget for area of responsibility.
  • Attend a paid training program.

Qualifications

Education, Knowledge, Skills and Abilities Required:

  • Master's in social work, public health, or equivalent is required.
  • Three years leadership experience with non-profit or community based organization. 
  • Superior knowledge of healthcare and social service systems including NJ public benefit enrollment. 
  • Excellent understanding of the healthcare and social service systems including processes for enrolling in public benefits.
  • Ability to analyze reports and use data to drive performance results.
  • Ability to exercise judgment in the application of professional services.
  • Excellent oral and written communication skills.
  • Ability to carry out written and oral instructions.
  • Passion for improving the lives of low-income individuals and families.

Education, Knowledge, Skills and Abilities Preferred:

  • LSW/MSW preferred.
  • Must be proficient in MS Word and computer data entry.
  • Prior experience as an outreach worker is a plus.
  • Knowledge of the resources and community-based organizations across New Jersey is a plus.

Starting Minimum Rate

Minimum rate of $113,609.60 Annually

Some jobs may also be eligible for performance-based incentives, bonuses, or commissions not reflected in the starting rate. Certain positions may also be eligible for shift differentials for work performed on evening, night, or weekend shifts.
