Overview

The Cardiogenic Shock and Percutaneous Ventricular Assist Device (pVAD) Coordinator, APN is an expert and patient advocate in the management of patients requiring advanced heart failure therapies including percutaneous mechanical circulatory support. This position manages a group of clinically complex or resource intensive patients. Prior pVAD experience required.

Responsibilities

1. Assessment, management and oversight of care of patients with advanced heart failure and/or cardiogenic shock; many of whom require intensive management including advanced heart failure therapies and mechanical circulatory support. 2. Prescription, initiation of, and up-titration of heart failure medications and treatments; many of which require prescription/ documentation by an APN or MD, including prescribing, titrating, and close monitoring of oral anticoagulants. 3. Screen referrals, communicate with physicians regarding new MCS patients 4. Provide coordination of care with other disciplines, other physicians, and other programs for complex group of MCS patients 5. Collaborate with ICU RNs, APPs and physicians (CTICU and MICU/CCU) on care of MCS patients and troubleshooting the device. 6. Demonstrates advanced assessment skills, within clinical specialty, providing evidence-based treatment interventions, to achieve positive outcomes for patients and families. 7. Participates in the evaluation and screening of patients for ventricular support in conjunction with the multidisciplinary team. Provides psychosocial and emotional support to patient and family; 8. Manages the care of the high risk percutaneous ventricular assist device (pVAD) patient across the continuum (acute care inpatient, clinic follow ups, community setting) in collaboration with the multidisciplinary team. 9. Provides clinical guidance and leadership through participation and/or representation at departmental and hospital meetings, committees, councils, task forces and multidisciplinary rounds, as needed. 10. Participates in strategic planning and development of the pVAD program and cardiogenic shock team in alignment with long term development goals of the Heart and Vascular Hospital/ HMH network. 11. Serves as a resource and support person for percutaneous ventricular assist device patients and their family members both in the acute care setting and in the community. 12. Coordinates the development of evidenced-based practice protocols and standards of care that include interventions and treatments to achieve desired outcomes, including clinical, financial, and patient/family satisfaction. 13. Utilizes and disseminates relevant research findings through clinical practice, education or consultation. Participates in clinical research and publication specific to pVAD therapy 14. Maintains internal pVAD database which may include site entries for the cVAD Ventricular Assist Device industry based registry 15. Participates in research studies focused on new treatments within the pVAD therapy 16. Creates/reviews Quality and Data reports for morbidity and mortality of the pVAD program including length of stay, readmission rates and causations, and adverse events rates 17. Monitors for device recalls pertaining to pVAD equipment and creates response plan to address improvement 18. Works in conjunction with facility leaders to develop program specific guidelines and reviews annually for areas of improvement. 19. Lecture at various national and local meetings and represent HackensackUMC's percutaneous VAD program and APN model in the national HF arena. 20. Other duties and/or projects as assigned. 21. Adheres to HMH Organizational competencies and standards of behavior.

Qualifications

Education, Knowledge, Skills and Abilities Required: 1. Graduate of an NLN/AACN accredited program in nursing. 2. Minimum of 3 years of APN experience in Cardiology/Cardiac Surgery/Heart Failure. 3. Maintains clinical competencies through participating in continuing educational programs (contact hours per years), and clinical practice as mandated by Board of Nursing and national certification body. 4. Strong organizational and computer skills. 5. Completion of a Masters Program in Acute Care NP.

 

Licenses and Certifications Required: 1. Current valid NJ State Professional Registered Nurse license. 2. Maintains certification as APN in New Jersey. 3. Maintains National or Board Certification as appropriate in area of specialty. 4. Completion of Abiomed training. 5. American Heart Association Basic Life Support Healthcare Provider and Advanced Cardiac Life support certification. 6. Collaborative agreement with physician(s) actively on staff who agree to act as the Collaborating Physician(s) of Record as per the APN Act and Regulations

Starting Minimum Rate

Minimum rate of $131,144.00 Annually
